摘要:
目的 探讨多种油脂肪乳(SMOFlipid)对老年胃肠肿瘤病人术后的影响。
方法 将55例胃肠道术后需肠外营养支持至少5 d的老年病人,随机分为对照组和研究组。分别给予两组病人等热量的标准营养支持,其中对照组采用200 g/L中长链脂肪乳,研究组采用SMOFlipid。分别于术前、术后检测两组病人各项生化、炎症及免疫指标。
结果 术后第6天研究组病人的淋巴细胞亚群(CD3+、CD4+)、免疫球蛋白(IgG、IgA)及总蛋白水平均显著高于对照组(t=2.35~4.46,P<0.05);而C反应蛋白水平及白细胞计数明显低于对照组(t=4.23、4.55,P<0.05)。
结论 SMOFlipid能显著改善老年胃肠道肿瘤病人术后的免疫状态,并抑制机体过度炎症反应,对改善病人术后机体状态有益。
Abstract:
Objective  To investigate the effects of SMOFlipid (a mixed lipid emulsion containing 30% soybean oil,30% medium chain triglyceride,25% olive oil, and 15% fish oil) on postoperative recovery of elderly patients with gastrointestinal cancer.
Methods  A total of 55 elderly patients who required at least 5 d of parenteral nutrition support after gastrointestinal operation were randomly divided into study group and control group. The two groups were given isocaloric standard nutrition support. The control group was given 200 g/L medium- and long-chain lipid emulsion, while the study group was given SMOFlipid. The fasting blood samples were collected from the patients in the two groups to determine the biochemical, inflammatory, and immune parameters before and after operation.
Results  At 6 days after operation, the study group had significantly higher levels of CD3+, CD4+, IgG, IgA, and total protein and significantly lower level of C-reactive protein and white blood cell count than the control group (t=2.35-4.46,P<0.05;t=4.23,4.55;P<0.05).
Conclusion  For elderly patients with gastrointestinal cancer, SMOFlipid can significantly improve postoperative immune status and inhibit excessive inflammation, so it is beneficial to improve postoperative status of patients.
