摘要:
目的 研究三维斑点追踪技术(3D-STI)评价结缔组织病(CTD)病人心肌应变的临床价值。
方法 选择30例CTD病人作为CTD组,30例健康志愿者作为对照组。二维超声模式下测量两组左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、每搏输出量(SV)、左心室射血分数(LVEF),采用TOMTEC软件分析心尖四腔心全容积图像,获得心肌整体应变参数,包括左心室整体纵向应变(GLS)、左心室整体圆周应变(GCS)、左心室整体径向应变(GRS),并进行比较。
结果 CTD组LVEDV、LVESV、SV、LVEF与对照组比较差异无显著性(P>0.05);GLS、GCS、GRS均低于对照组,差异有统计学意义(t=2.08~3.27,P<0.05)。
结论 3D-STI可早期检测CTD病人的左心室心肌应变改变,具有较高的临床应用价值。
Abstract:
Objective  To investigate the clinical value of three-dimensional speckle-tracking imaging (3D-STI) for evaluating the left ventricular global myocardial strain in patients with connective tissue disease (CTD).
Methods  A total of 30 patients with CTD were selected as the observation group, and 30 healthy volunteers were recruited as the control group. The following parameters were measured by two-dimensional ultrasound: left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), stroke volume (SV), and left ventricular ejection fraction (LVEF). The images of apical four-chamber view were analyzed by the TOMTEC software to determine left ventricular global longitudinal strain (GLS), global circumferential strain (GCS) and global radial strain (GRS). All the parameters were compared between the two groups.
Results There were no differences in LVEDV, LVESV, SV, and LVEF between the CTD group and the control group (P>0.05). The CTD group showed significantly decreased GLS, GCS, and GRS compared with the control group (t=2.08-3.27,P<0.05).
Conclusion  3D-STI is a reliable tool for evaluation of left ventricular global myocardial strain in patients with CTD and holds promise for clinical application.
