摘要:
目的 探讨囊性小肾癌的MRI表现与Bosniak分级之间的相关性。
方法 选择2013年2月—2014年2月入住我院的肾脏囊性病变病人85例,将其中经CT检查结合Bosniak分级诊断的16例囊性小肾癌病人作为对照组,依据MRI模拟Bosniak分级诊断的18例囊性小肾癌病人作为观察组。计算MRI模拟Bosniak分级诊断囊性小肾癌的有效率,并比较两组病人的Bosniak分级差异,以及MRI和CT两种检查方法的Bosniak分级差异。
结果 观察组检出的18例囊性小肾癌病人,均经病理检查确定为囊性小肾癌,有效率为100%;而对照组检出的16例囊性小肾癌病人,经病理检查确定为囊性小肾癌12例,有效率为85.71%,两组比较差异有统计学意义(χ2=5.100,P<0.05)。两组病人Bosniak分级比较差异无显著性(P>0.05)。MRI和CT两种检查方法的Bosniak分级比较差异有显著性(u=19.258,P<0.05)。
结论 MRI结合Bosniak分级诊断囊性小肾癌具有一定的可行性,与 CT检查比较,可以提高恶性病变的诊断有效率,避免病人错过治疗时机。
Abstract:
Objective  To investigate the correlation between magnetic resonance imaging (MRI) findings and Bosniak classification in small cystic renal cell carcinoma.
Methods  A total of 85 patients with renal cystic lesions who were admitted to our hospital from February 2013 to February 2014 were enrolled in this study. Of the 85 patients, 34 who were diagnosed with small cystic renal cell carcinoma were divided into control group (diagnosed by computed tomography (CT) and the Bosniak classification system, n=16) and observation group (diagnosed by MRI simulation and the Bosniak classification system, n=18). Pathological examination was used to confirm the diagnosis and the diagnostic efficiency was calculated for the two groups. Bosniak classifications were compared between the two groups and between MRI and CT.
Results  As confirmed by the pathological examination, the observation group had a significantly higher diagnostic efficiency than the control group (100%(18/18) vs 85.71%(12/16), χ2=5.100,P<0.05). There was no significant difference in Bosniak classification between the two groups (P>0.05). However, there was a significant difference in Bosniak classification between MRI and CT (u=19.258,P<0.05).
Conclusion MRI combined with the Bosniak classification system has a certain feasibility. Compared with CT examination, it can improve the diagnostic efficiency of malignant lesions and prevent patients from missing the timing of treatment.
